Before mailing a form to our office, please read all the instructions carefully. If you wish to have a time-stamped copy, you must come to the office or include a self-addressed, stamped envelope if you mail the form. Some applications require that payment of a fee be submitted before the application can be processed. The forms can be submitted either in person or by mail at the following address:
